3)If you want your item insured or not (Foreign orders cannot be insured.) If you do not tell me what you want, I may automatically insure larger US orders. If you don't insure your order the loss will be yours. 

4) Your Paypal email if it is different from the email you are sending the order from.

5)Items may be removed from packaging to save on shipping costs-please state if you do not want your packaging removed.
